Panchkarma — from the Sanskrit Pancha (five) + Karma (actions or procedures) — is the classical Ayurvedic system of five bio-purification therapies described in the Charaka Samhita and Sushruta Samhita. It is the most comprehensive system of internal body purification and rejuvenation in the world's medical traditions — designed to systematically eliminate accumulated Ama (metabolic toxins) from the deepest tissue levels and restore complete doshic balance.
Unlike symptomatic treatments — which manage disease without removing its root cause — Panchkarma works at the level of the Dhatus (body tissues) and Srotas (body channels) to eliminate the underlying doshic imbalances that produce disease. This is why Panchkarma produces lasting results for chronic conditions that have not responded to conventional or symptomatic treatment.

Virechana is the classical Ayurvedic medicated purgation therapy — designed to eliminate aggravated Pitta Dosha and Ama from the liver, small intestine and blood (Rakta Dhatu). It is preceded by 3–5 days of Abhyanga + Swedana and internal Snehana (medicated ghee consumption) to fully saturate and mobilise toxins before their elimination.
At Ayutirth Ayurveda, Virechana is performed using classical Pitta-reducing purgative preparations — typically Trivrit Lehya or Avipattikara Churna — calibrated to the patient's constitution and the degree of Pitta imbalance.
Basti is described by Acharya Charaka as the single most important treatment for Vata Dosha — and since Vata governs all movement in the body (including nerve impulses, joint function, peristalsis and elimination), Basti has the widest therapeutic range of any Panchkarma procedure. There are two classical types: Niruha Basti (decoction enema) and Anuvasana Basti (oil enema) — alternated in a prescribed sequence called Karma Basti or Yoga Basti.
At Ayutirth Ayurveda, Basti formulations are prepared fresh for each patient — using classical Kashayam, medicated oils and Ghrita in the correct proportions for the patient's specific doshic and disease presentation.
Nasya is the Panchkarma procedure for eliminating aggravated Doshas from the head, brain and sense organs — administered through the nasal passage, which classical texts describe as the Shirasdwara (gateway to the head). Medicated oils, herbal powders or ghee are instilled into the nostrils after local Abhyanga and steam fomentation of the face and neck.
Different types of Nasya are used at Ayutirth Ayurveda depending on the indication — Virechana Nasya (cleansing), Brimhana Nasya (nourishing) and Shamana Nasya (pacifying) — using classical preparations including Anu Taila, Brahmi Ghrita and Shadbindu Taila.
Vamana is the Panchkarma procedure for eliminating aggravated Kapha Dosha from the stomach and upper respiratory tract via controlled therapeutic emesis. It is the primary treatment for Kapha-dominant chronic diseases — particularly respiratory conditions, obesity and metabolic disorders. Vamana is performed using classical emetic preparations including Madanaphala Pippali and Yashtimadhu Kashayam.
Vamana is the most strictly indicated of the five Karmas — requiring careful patient selection, full Poorvakarma preparation and precise execution. Dr Solanki evaluates eligibility thoroughly at consultation before recommending this procedure.
Raktamokshana is the fifth Karma — the classical blood purification procedure for eliminating Pitta-vitiated Rakta (blood) in skin, inflammatory and vascular conditions. Panchkarma goes deeper — it physically eliminates accumulated Ama and aggravated Doshas from the body tissues through specific bio-purification procedures (Virechana, Basti, Nasya, Vamana, Raktamokshana). The analogy often used in classical texts is this: oral medicines work like detergent washing clothes while they are being worn. Panchkarma works like removing the clothes entirely, washing them thoroughly and putting them back clean. Panchkarma is indicated when oral treatment alone is insufficient for a chronic or complex condition.
Keralian Panchkarma refers to the advanced Panchkarma system developed in Kerala — incorporating specialised external therapies (Shirodhara, Navarakizhi, Pizhichil, Elakizhi, Kati Basti) alongside the classical five Karmas. Keralian training requires extended hands-on clinical apprenticeship at recognised Kerala institutions — it cannot be acquired through short workshops. A complete programme — Poorvakarma (3–7 days) + Pradhana Karma (1–7 days) + Paschatkarma (7–14 days) — typically spans 14–28 days in total. For Poorvakarma (Abhyanga + Swedana preparation), most patients can continue working — sessions are completed in the morning and you can resume light work in the afternoon. For the Pradhana Karma (main procedure day — e.g., Virechana, Basti), 1–3 days of rest at home is required. Paschatkarma involves dietary restrictions and reduced activity rather than full rest.
