Agnikarma (अग्निकर्म) is one of the eight para-surgical procedures described in classical Ayurvedic texts — specifically in Sushruta Samhita. It involves the application of controlled therapeutic heat to precise anatomical, Marma and trigger points.
Unlike superficial heat therapies such as hot packs or infrared lamps, Agnikarma delivers therapeutic heat directly to the affected tissue — be it joint margins, nerve roots, tendon insertions or periarticular trigger points — with precision that cannot be achieved by any surface application.
Sushruta declares Agnikarma to be superior even to surgery and other Shalya procedures for certain conditions — particularly those of the joints, tendons, muscles and nerves. The classical text states: "Diseases which are not cured by medicines, not cured by Shastra (instruments), are cured by Agnikarma."

Lumbar disc herniation — popularly called slip disc — is a leading cause of severe back pain and leg pain in adults. Agnikarma applied to precise spinal and nerve root points produces rapid relief without surgery.
A slip disc occurs when the soft nucleus of an intervertebral disc protrudes through the tough outer annulus fibrosus, pressing on adjacent nerve roots and producing severe back pain, radiating leg pain, numbness, tingling and in severe cases, weakness of the leg muscles.
In Ayurveda, slip disc corresponds to Kati Vata with Gridhrasi — a severe Vata disorder affecting the lumbar spine and sciatic nerve. Agnikarma, by applying precise therapeutic heat to the specific spinal and nerve root points, directly pacifies the aggravated Vata, reduces local inflammation and re-establishes normal nerve function.

Sciatica — known as Gridhrasi in Ayurveda — is characterised by sharp, shooting or burning pain radiating from the lower back through the buttock, thigh, calf and foot along the sciatic nerve pathway.
Sciatica is one of the most painful and functionally disabling conditions that brings patients to Ayutirth Ayurveda. The radiating nature of the pain — shooting through the leg, often accompanied by burning, numbness, electric sensations and muscle weakness — makes normal walking, sitting and sleeping extremely difficult.
In Ayurveda, Gridhrasi (sciatica) is a Vatavyadhi — a pure Vata disorder characterised by pain following the pathway of the sciatic nerve. Agnikarma is the primary para-surgical treatment for Gridhrasi described by Sushruta.
Unlike oral nerve pain medications, which suppress nerve signalling systemically, Agnikarma for sciatica acts locally on the precise points of nerve irritation — deactivating ectopic pain signalling at the nerve, reducing perineurial inflammation and restoring normal nerve conduction.

Each Agnikarma session at Ayutirth Ayurveda is conducted personally by Dr Harshad Solanki in a clean, professional clinical environment in Gota, Ahmedabad.
Dr Solanki performs a comprehensive clinical examination — range of motion, tenderness, neurological examination including reflexes, straight-leg raise, sensation and motor strength. All available investigations (X-ray, MRI, nerve conduction studies) are reviewed.
The specific points to be treated are carefully identified and marked — Marma points, trigger points within periarticular muscles, joint margins, paravertebral muscles, facet joints, nerve root exit points and the complete sciatic nerve pathway as applicable.
The treatment area is cleansed and a light application of medicated Ayurvedic oil is made. For spinal and sciatic conditions, a brief warm oil massage to the lower back and posterior thigh is performed immediately before the Agnikarma application.
The selected Shalaka (instrument) — heated to the precise therapeutic temperature — is applied to each identified point with a controlled, brief contact lasting a fraction of a second. The procedure takes 10–15 minutes for knee pain; 20–30 minutes for slip disc and sciatica.
Medicated Ayurvedic ghee is applied to all treated points. The patient rests for 15–20 minutes. Small reddish marks at each treatment point typically heal within 3–5 days and leave no permanent marks.

Yes — for most patients with disc herniation who have not yet developed progressive neurological deficit (foot drop or loss of bladder/bowel control). Agnikarma applied to the specific spinal and nerve root points reduces nerve root inflammation, relieves paraspinal muscle spasm and addresses the Vata imbalance underlying disc herniation.

Most patients experience meaningful reduction in the burning, shooting radiating pain within the first 2–3 Agnikarma sessions. The improvement in leg pain often begins even before the back pain fully resolves. By sessions 5–7, the majority of patients find they can walk, sit and sleep significantly more comfortably.
The number depends on the condition and its severity. As a general guide: knee pain Grade 1: 3–5 sessions; Grade 2: 5–7; Grade 3: 7–10; Grade 4: 10–14. Slip disc: 7–12 sessions. Sciatica: 8–14 sessions. Sessions are spaced one week apart. Dr Solanki reviews progress at every session and adjusts the plan accordingly.
The procedure causes a brief, momentary sensation of sharp heat at each treatment point — lasting only a fraction of a second per point. Most patients find it significantly more tolerable than they expected. No sedation is required. After the session, the treated area may feel warm or slightly tender for 24–48 hours. Most patients are able to walk out of the clinic, drive home and resume light daily activities.
Yes — and this is our standard approach at Ayutirth Ayurveda. For knee pain, Agnikarma works most effectively when combined with Janu Basti and Pinda Sweda. For slip disc and sciatica, the combination of Agnikarma and Kati Basti in the same session is significantly more effective than either alone.
Agnikarma is generally safe for elderly patients and can be particularly valuable for this group who often cannot tolerate surgery or long-term NSAID use. Diabetic patients with poorly controlled blood sugar require special caution. Dr Solanki evaluates each patient thoroughly before proceeding and for well-controlled diabetics, Agnikarma can be performed safely with appropriate precautions.
